Registration Information

The Maryland State Department of Education - Office of Child Care, requires all licensed providers to have the following forms on file for each child in their care.

You will only need to fill out these forms once, and then update them yearly. Shot Records will need to be updated as your child is given them. I keep these completed forms on file here, and will remind you when it is time for updating.

Steps to Register your child(ren) at Carol’s Crunch Care:

  1. Call Carol to set up an interview to meet her, her children, and to see her facility. You may want to make a list of questions to bring.
  2. Fill out the forms. You can download them off of this website, or Carol can mail them, or give them to you when you go for the interview. At the interview, Carol will give you her contract for you to read over and sign.
  3. Return complete forms to Carol by mail, email, or drop them off at your convenience.

  4. Call Carol when you need to bring your child(ren) to Carol’s Crunch Care. Remember, that if you call 12 hours or more in advance you will be paying the discounted rate.

    This is the information that the provider will need to get in touch with you in the event an emergency takes place. Or, if the parent/guardian is having an emergency, who the people will be to take care of your child, when it is time for them to be picked up from Day Care. The back only needs to be filled out, if your child has a condition that might require emergency medical care.
    This form needs to be filled out by the parent/guardian, and the pediatrician. It asks questions about your child’s medical history.
    This is the Lead Screening form. It verifies that your child has been screened for lead.
    This only needs to be filled out if your child is taking medication that the provider will need to administer.

    This form will give your provider more information about what your child can do, needs help with, special routines, etc.

    This is the form that your pediatrician’s office will fill out to verify that your child is up to date on all of their immunizations.
